Returning to campus – self declaration form

Colleagues are asked to fill in self declaration form when requested by their manager.
Managers and colleagues are asked to ensure that they and their team members fill in the self-declaration form before returning to work on campus.
Please ensure that you access the most recent version of the form, which is subject to change as Government and University guidance changes. It can be found here:
The form can also be found in point 2 on the Guidance for Managers page of the Campus reopening and corporate support page of StaffNet.
There is no need to fill out the form again if you have already completed one.
Please note that all staff who are able to work from home should continue to do so until at least 1 September 2020. If you are required to come back to campus you will be notified by your line manager.
